I play out my games, I have those settings, and I don't remember ever getting those messages. Of course, I have injuries set to 'very low', so I may not be the best person to check this.

I have seen it now!
Set up a test league.
Injury Frequency - Very High
Manager Options - Exit Auto Play...
No limit on both when a player is injured and DTD injury.
Played out the games and a DTD injury notification occurred as I selected the "leave game" button after finishing a game.
Appeared as a pop-up box over the top of the profile of the player who is DTD. (Just the same for when a player is injured).
No message appeared in my News section (a message would have for an out and out injury).
But an alert did happen (and does) if you play out games.
(BTW I didn't do this to prove a point...I was just curious and thought it might be helpful....and I had to play OOTP for 25 games before a DTD injury happened...even on very high injury frequency....but I didn't mind
