Failure to enforce the rules as written. Most notably the strike zone, but also blocking access to bases without the ball, phantom double plays.
Also balks. The existence of the rule. The inconsistency of its application. The fact that some of the rule outlaws actions that are physically impossible.
Aggressive umps. This season I've seen a major league ump have to be restrained from going out to the mound by the catcher.
