Do you have it set to delete players who never reach the majors? I believe the game reuses those player id numbers, which then means that if your scout scouted the player that was deleted, when his player id is used again it shows the scouting for the old deleted player. Haven't tested that or put much thought into it, but was always my best guess.

The second part, I believe your head scout (whoever it was when you first took over the team) scouts every player there is anywhere... then of course the report is near useless unless he keeps up with it. So if you took over the team 7 years ago and it scouted some poor schmuck in Japan (or the winter league, or A ball or whatever), and you never ended up scouting him again somehow... if the player moves over to MLB this season your scout is giving you the 7 year old information he has on him.
