1) - 6) Haven't seen any.

7) - had one in an OOTP5 game I played today. Pretty sure I saw one or two in OOTP4 as well.

8) Can't gauge based on the limited # of games I've played with OOTP5. If I had to guess, I would say this hasn't changed.

9) Haven't seen any. The strike-em-out-throw-em-out isn't even possible if the human-controlled team is at bat, because you're not allowed to try and steal if there are two strikes on the batter (just like in OOTP4).
