I wonder if the problem shows up variably depending on when you have the amateur draft scheduled. Mine is on June 15, and the FA was signed away early in the preceding December.

I'm sure the timing of the draft is not the issue. In OOTP 2006 (and I believe in OOTP 2007 as well, though I only patched recently and haven't checked thoroughly): if you lost a type A free agent to a team with one of the first 15 picks, you would receive a message saying that you were receiving a compensations pick in the 1st round, but since the first 15 picks are protected, the compensation pick wouldn't actually be awarded; you'd get nothing. If the team that signed your FA was in the bottom half of the league, that is likely the issue.
Another possibility is that the team that signed your type A free agent also signed a type A free agent from another team. In that case, their 1st round pick might have gone to the other team (and I think in that case, you don't even receive a message telling you that compensation has been awarded to you).
Because the current OOTP draft structure does not accommodate a sandwich round, it is difficult for the game to provide a flexible and fair compensation system. I still use it, but I accept that I'll lose some free agents for nothing, and that the game won't model MLB compensation completely. The compensation system won't be changed in the game soon, according to the response to this thread (and thanks, bp_, for the reply):
