The league creation wizard walks you through all of the basic questions you need to answer when creating a league. There is one for fictional, one for historical, etc. Once you get past that (and the wizards are very easy and comprehensive), you can tweak the settings before you actually start the league.
It won't necessarily walk you through something as specific as a partial MLB/partial fictional league, but that's what the smart people on the forum are for!
To answer another question above, since you can totally add/delete/associate/unassocitate teams and leagues with ease, it's almost impossible to screw up a league setup to the point where you cannot easily fix it. (Nothing's perfect, of course, but it's close.)
