Steve, most any setup will randomly do this when trading players. In an online league, you can get it do do it everytime. A league member can simply offer a trade to an AI team and export. If the AI team accepts, the league member can simply confirm the trade and export. In the commissioner's game after importing teams, players involved in that trade will be in limboland.

Also, in commish mode, simply editing a player's team in his profile page will throw him into limboland.

I'm not sure, but it could be related to the fact that there is no "true" "Designated for Assignment" as the current game treats it as one in the same with "Waivers"(which is another bug).
