Drug testing
Each month a team is randomly chosen, and each player from that team (all levels - major and minor) are run through a random number generator. The percertanges are different for different types of players, ie a 37 year LF'er who hits 73 homeruns is more likely to be on the juice than a 22 year old .220 hitting shortstop. The percentages are low enough that not many players test positive (I ran a test of this for 9 seasons worth of testing, very few tested positive).
If a player were to test positive, he faces the penalty for the given drug. Once he finishes his suspension, he is tested again to see if he is sober.
