You've covered loads of tiebreakers for sure.
FIFA have never revealed the format for the way they rank the teams from 1-16, but this is what I've gathered from analysing the actual positions.
All knockout games count 3 points for win irrespective of when it happened (normal, extra, or penalties), and 0 points for a loss (and goals scored/conceded in 120 minutes are all counted).
Then it gets complicated. In 1998, it seems they vaulted a team losing on penalties to the top of the round bracket (England's loss to Argentina on penalties moved them up to 9th from 13th I think), but in 2002 shootout losses didn't count.
And yes, each round is bracketed off so Round of 16 losers finish 9-16, quarterfinal losers 5-8, and obviously the 1st to 4th are self-explanatory.
