It has been reported, although I'm not sure if this is one of the issues being addressed in the patch. I've had the same issues you have. Runners on 2B not drawing a throw at home on simple base-hits to the outfield. Runners stealing third like the world was ending, etc.
Your second issue sounds like a deficiency in the "token system" the PxP uses, where there are certain lines called for in certain situations. This is also how you have a player getting turned around, losing the lights, falling on his ass, and making A STUPENDOUS CATCH!!!!
Hopefully both issues will be addressed.
