I've even re-installed a complete fresh new version OOTP OU and get the same problem.

So the curse of the 8s strikes again!!! Now, a decade later, my 2008 draft class has the EXACT same issue -- csv is updated, development is done. Draft was handled the same way I do it every 2 weeks...

Had an odd problem happen twice in our draft (and possibly more that I just haven't heard.

A GM does not have a list or any auto-select buttons checked, but the utility picks for him anyway.
