There is a Facegen bug that has existed through a few different iterations of FHM. It's very inconsistent, which is the one I do believe you're describing. It may appear in some games and not others....

That sounds more like something didn't install correctly. Do a reinstall, or if you are on Steam, Verify the Game Cache (and if that is the case, make sure Steam is running in Administrator Mode).

It wasn't so much a development problem, the problem is the save files for FHM get larger over time and so when people are using OneDrive for personal things and then the game starts getting uploaded...
