If I may ask, at what point in the season is the player being placed on revocable waivers?
The only revocable waivers which should exist in-game are for trading a player after the trade deadline. In regards to DFA-ing a player, irrevocable waivers should only exist for outrighting a player to the minor leagues since, as mentioned, release waivers are a formality in real life and thus can be effectively skipped in-game (except perhaps for historical or fictional leagues not using finances).
OOTP really should differentiate between the waiver types (e.g. optional, outright, trade, and release) since that would mean better precision and allow for better determination of what kind of waiver move might be causing problems.
